About this resource

I like to send home a family survey either at our meet the teacher night or on the first day of school. I know parents are already asked to fill out so many forms. However, it is so important for teachers to have a chance to learn a little more about the family and the child.

This survey asked parents to share student strengths and weaknesses along with their child’s overall feeling about school. There are places for parents to share any additional information that a teacher might find helpful.

When printing the form, you will have two choices for the back.

The first option contains a section a the bottom where parents can sign permission for candy or other small snacks in the classroom. I like having this on the back of the survey. I keep the survey and put it in my binder, having the snack section on the back allows me to find it quickly if needed.

If you school does not allow teachers to hand out any types of food, the next page is the one you will want to use.
